What do I need to bring to my appointment?

  1. Completed paperwork - Download Registration , Financial Policy forms

  2. Insurance card(s)

  3. Picture ID (ex. drivers license)

  4. Any other reports from testing pertinent to your visit with the doctor

  5. A list of medications you are currently taking
     

What do I need if this is a workers compensation case? 

For workers compensation cases, a  letter of authorization is required. The letter of authorization must be on letterhead from the employer or the worker’s comp insurance. The letter must state that the patient was injured on the job and that they will be covered under worker’s comp insurance. The authorization letter must include the following:

 

  1. Name and address of worker’s comp insurance

  2. Telephone number of worker’s comp insurance

  3. Claim number

  4. Part(s) of the body to be treated

  5. Date of injury

  6. Name of work comp adjuster (if available)

 

Authorization letters for worker’s comp injury may be faxed to us at 980.321.5238 

 

How do I get my prescription refilled?

Because of the medications we prescribe require physician approval you will need to call our offices in order to secure a refill.  To prevent running out of your medication please call us BEFORE you run out as it may take time to get your medication refill.

Running late or need to cancel?

If you're running late please call us. We will make every effort to still see you or to reschedule for later in the day if we have an opening to avoid cancellation. 


If you need to cancel please give us as much notice as possible but there is a 24 hours minimum notice required if you desire to avoid the cancellation fee, charges incurred and those not covered by insurance.

Co-Pays

Co-pays are expected at time of service unless other arrangements have been made in advance. Please know that the patient is ultimately responsible for all charges incurred and those not covered by insurance.
